BASKETBALL FORMS YOU MAY NEED - FIRST GAME MONDAY, DECEMBER 6, 2021 AT HOME

The Transportation Waiver for Athletics form is utilized when you personally will be transporting your student-athlete from an away game rather than having them ride the school bus. This form must be filled out and handed to the Coach prior to taking your student-athlete from an away game.

The Permission to watch a Home Basketball Game form must be turned into the office for the Site Supervisors for our Home Basketball games. Students can either stay directly after school until the game begins or go home and return with parental permission to view the game. A new form must be filled out for each game.

CONNECTICUT CHILDREN’S HOSPITAL “PJ DAY”

Each year, Connecticut Children’s holds a PJ Day where students, staff, and families may donate any amount to contribute towards the fight against cancer and wear their pajamas to school. PJ Day this year is set for December 10, 2021. Our whole district is once again taking part in this meaningful CCMC PJ Day fundraiser. This fundraiser helps children who are fighting cancer. Donations can be sent to homeroom teachers. The Student Council thanks you for your donation. Let's make this year the biggest donation year ever!

A NOTE TO THE OFFICE

Our Note to the Office form is convenient! Print it, send one of them in with your student about an early pickup need, a previous absence, a tardy, or vacation. If your student is leaving early, we can prepare them with a pass to be in the office five minutes before your arrival after they leave class without an interruption.

JOSTENS YEARBOOKS

Let’s not forget to order those yearbooks for your student. They are only $37 (this could increase in the new year). You can also place 8th-grade grad ads at the same time as placing your yearbook order. There are only so many pages allotted for Grad Ads so get those orders in before space runs out. Simply log onto www.Jostensyearbook.com and follow the prompts.
