A MESSAGE FROM THE HEALTH OFFICE, DISTRICT NURSE SUPERVISOR NONIEWICZ

Our goal is to keep your child safe and in school this year. In order to do this, we are required to “contact trace” all close contacts with COVID positive individuals. This contact tracing may result in the need to quarantine certain individuals based on their vaccination status. In an effort to provide you accurate information and to expedite our contact tracing process, it would be helpful to have Covid19 vaccine information on file with the nurse (this is not a requirement, but it helps to identify those students who need to quarantine versus those students who can remain in school in the event that your child has been identified as a close contact). Fully vaccinated students who are close contacts can remain in school if they are symptom free however we need the proof of vaccination on file.

If your child is fully vaccinated could you please provide the school with proof of their Covid-19 vaccine. Acceptable forms of verification are:

CDC COVID-19 Vaccination Record Card or photo of the Vaccination Record Card;

Documentation from a health care provider or electronic healthcare records; or

State Immunization Information record.

The proof can be sent to cnoniewicz@colchesterct.org

Call with any questions to 860-537-3511.
