Back to School Health Record Forms

Reminder from the Colchester School Nurses:

As we prepare for a new school year, we ask all families to ensure their child's health records are up to date before the first day of school.

If your child has a medical condition that requires medication, treatment, or accommodations during the school day, please submit all required medical forms and updated healthcare provider orders as soon as possible. This includes, but is not limited to:

  • Medication administration forms
  • Asthma Action Plans
  • Allergy/Anaphylaxis Action Plans
  • Diabetes Medical Management Plans
  • Seizure Action Plans
  • Any other physician orders or health-related documentation needed for school

Updated forms allow our school nurses to safely prepare for your child's health needs and ensure that medications and services are in place when school begins.

Please submit completed forms to your child's school nurse or the Health Services Office prior to the start of the school year. If your child no longer requires medication or medical services at school, please notify your school nurse with the updated notes from their prescriber. You can set up a time with your school nurses for medication drop off prior to the start of school.
